I declare that I am joining the FM WORLD Club and I will participate in it under the terms and conditions of the Marketing Plan, the FM WORLD Club Regulations and the Code of Ethics, I have received a copy of the Marketing Plan, the FM WORLD Club Regulations, the Code of Ethics, and I am familiar with their contents, which I accept in full.
I undertake to comply with the provisions of the FM WORLD Club Regulations, the Marketing Plan and the Code of Ethics during the term of membership in the FM WORLD Club.

In case of lack of consent I shall be present on the Network website only under the Business Partner’s (Distributor’s) Number assigned. I declare that I will have the Seller status. I will run economic activity in the scope of direct sell of Products and providing of advertising services of the FM World Network and Products.
I declare that I shall operate only on the User status and to purchase FM WORLD Products for my own (or my close ones’) needs only. I further declare that regarding these Products I shall neither render direct sales nor run economic activities.
I consent to the processing of my personal data and the data necessary for billing and cooperation by FM GROUP World Artur Trawiński spółka z ograniczoną odpowiedzialnością, sp.k. with the registered office in Wrocław, ul. Żmigrodzka 247 (KRS: 0000268185, NIP: 8951870093), hereinafter “FM WORLD” or FM World Qatar spółka z ograniczoną odpowiedzialnością, sp.k. with the registered office in Wrocław, ul. Żmigrodzka 247 (KRS: 0000636342, NIP: 8952093148), hereinafter “FM WORLD Distribution” or a FM WORLD Branch, in the Business Partners (Distributors) database, created within the organization of an international Network, for the purposes of implementing the provisions of this Agreement and its appendices. I give my consent for the transfer of my personal data by FM WORLD to FM WORLD Branches. FM WORLD is the administrator of the database, which includes personal data of all registered Business Partners (Distributors), and FM WORLD Distribution is the administrator of the database, which includes personal data of the registered Business Partners (Distributors) of FM World Qatar. The purpose of personal data collecting and processing is: registration in the FM WORLD Club, which enables purchasing and organisation of the Network, financial settlements and documentation of business transactions, calculating points, Trade Discount and Remunerations, as well as proper documentation, remaining in contact with other Business Partners (Distributors) and their Groups, to promoting the most active Business Partners (Distributors), implementation of cooperation in terms of providing Business Partners (Distributors) with goods and services, in the sales of intermediate respectively FM World Qatar or a Branch, implementation of loyalty and incentive programmes. The recipients of the personal data may be: (a) other Business Partners (Distributors), (b) parties personally or financially associated with FM World Qatar, (c) potential customers of FM WORLD, FM World Qatar or a Branch and third parties who declare willingness to join the FM WORLD Club, (d) courier companies providing services to FM World Qatar, FM WORLD Branch or Business Partners (Distributors), (e) companies which organise Business Partner (Distributor) events for FM World Qatar or FM WORLD, (f) training companies, (g) advertising agencies, (h) PR companies.
